As part of your Bloomin’ Brands medical benefit, you have access to a well-being program that rewards you for completing Health Actions.
Team Members and spouses enrolled in the My Health Savings Plan or Choice HRA plan can earn Health Rewards.
How it works
Complete health actions during each plan year to earn rewards deposited into your Health Reimbursement or Health Savings Account.
The Rally Health Survey is the first step to access and earn rewards.
The Health Survey provides you a snapshot of your current health age and recommendations on actions you can take to improve or maintain your health. Complete the Rally Health Survey by December 31.

If your spouse is enrolled in your UHC health plan, they can earn rewards, too! You’ll get an additional $150 reward deposited to your HRA or HSA* if your spouse also completes Health Actions under their own myuhc.com profile no later than December 31.
You can update your Health Survey throughout the year. You will not receive a second reward in the same year when you update your Health Survey. The Health Survey must be completed each year.

**Not available in the state of New York.
If you earn Health Rewards while you have single coverage, but then add dependents throughout the year, you will not receive an additional Health Reward for actions completed prior to your coverage change. Similarly, if you earn Health Rewards while enrolled in dependent coverage, but then change to single coverage, you will not be asked to return your original reward.
How to complete Health Actions

|Rally Health Survey||Log in to myuhc.com or the Rally app and take your confidential Health Survey.
You must complete the Rally Health Survey first to access and earn additional rewards. The survey must be completed each year.
|Annual physical||Receive your annual physical from your physician.
Your reward is deposited within 2-4 weeks after UnitedHealthcare (UHC) receives and processes your physician’s claim.
|Biometric screening||A biometric screening includes blood pressure, BMI (height and weight), glucose, and cholesterol screenings. Your Health Reward is earned upon participation confirmation from UHC. Please allow 2-4 weeks.
Lab Screening: Log on to Rally via myuhc.com to download your Quest Diagnostics Physician Results Form. Take the form with you to your appointment for your physician to complete and return to Quest Diagnostics.
At-Home Screening:** Log on to Rally via myuhc.com to request the At-Home screening test. Follow the steps included in the packet to complete this new screening option. At-Home kits must be ordered by December 1 to allow for shipping and processing time.
|Quit For Life Program||Take the first step to quitting tobacco by calling UHC at 1-800-985-3290.
Your Health Reward is earned following program completion.
|Real Appeal||Visit Real Appeal at bbi.realappeal.com to enroll. You must be at least 18 years or older to participate and have at least a Body Mass Index (BMI) of 23 to be eligible for the program.
Complete 9 sessions to earn your Health Reward.
|Maternity Support Program||Enroll during the first trimester of your or your spouse’s pregnancy to receive $100 in your funding account following enrollment.
Call the UnitedHealthcare Concierge to enroll.
|Quarterly Themed Missions||Quarterly themed missions are designed to focus on specific areas of your health. For 2021, missions focus on stress, exercise, nutrition, and financial wellbeing.
Complete up to 4 quarterly themed missions to earn the reward for this health action.
|Quarterly Themed Quizzes||Healthy quizzes are new for 2021 and are aimed at helping to teach you about your health.
Complete up to 4 quarterly quizzes to earn rewards for this health action.
|Quarterly Rally Challenge||Use a tracking device or the Rally app to count your steps and compete with others over the course of the Challenge. Challenges are 3-14 days in length depending on the specific challenge distance.
Complete up to 4 quarterly Rally Challenges to earn the reward for this health action.
|Personal Coaching||Get personalized well-being strategies with your own personal coach.
Complete the program and you’ll earn $$$ for your health funding account.
Health actions must be completed between January 1 and December 31 of the current year to earn rewards toward the current year’s program. Health actions completed after December 31 of the current year will apply to next year’s Health Rewards.
For My Health Savings (HSA) Plan participants, you must have an open HSA with Optum Bank within 60 days of earning any Health Reward in order to receive it.
Rewards are earned upon confirmation of completion from United Healthcare. Individual results are not shared with Bloomin’ Brands.
Spouses must complete their health actions under their own myuhc.com profile.
